Hi. Your design is very clever and it touch my poor creativity. Looking at it, I see that it combine 2 different adapter in only one dongle. The three switches are neeeded to choose witch one will be supplied and used. My experience make me be suspicious about the switches and their quality, so I have redo you design replacing your three switches with only three jumpers, everyone with two positions.
My opinion is that this will slow the operation to change from ISP to UART (and vice-versa) but this is an advantage. Another advantage is the minor space that the jumpers occupes respect the switches. I also can suggest to put a LED on the Vcc_ISP and another on Vcc_UART (but this is not important) connected to a common resistor to GND.
All the three jumpers will be on the left for ISP use and all on the right for UART use.
My silly suggestions are coming from the great ammiration about your work: my compliments.
